MUSKOGEE COUNTY, Okla. — The driver involved in a fatal Muskogee County crash that caused the death of an 18-year-old woman has been charged with first degree manslaughter.
We covered when the crash occurred on Sunday.
According to the Oklahoma Highway Patrol (OHP), 18-year-old Isabella Clay from Haskell was driving southbound on OK-72 near West Dozer Road around 1:00 a.m. on Sunday.
Authorities stated 35-year-old Levi Crawford from Tulsa was driving northbound on OK-72 before crossing the centerline and striking Clay’s car.
Law enforcement stated Clay’s car was pushed off the roadway and into a ditch, while Crawford’s car spun 180 degrees before eventually coming to a stop in the middle of the road.
Authorities said Clay was pronounced dead at the scene.
Court records document that as first responders and witnesses arrived to the scene, they stated Crawford showed signs of being under the influence of alcohol.
Court records show that a passing driver gave a statement to an OHP trooper, explaining he was driving southbound on the highway when he noticed the crash and decided to pull over.
The witness told authorities he approached the truck and helped Crawford out of the passenger side door because the driver’s side was jammed.
The affidavit writes the witness then stated “the guy from the truck stumbled around, then went to the other vehicle, where he sat down. The guy from the truck was noted as having a very strong odor of alcohol on his person and vehicle.”
Another witness statement recorded in the affidavit also noted Crawford smelled like alcohol.
Following the crash, Crawford was taken to the hospital and admitted for treatment before later being released.
According to court documents, a trooper arrived at the hospital to speak with Crawford.
The trooper stated he noticed Crawford smelled of alcohol and had “red, watery eyes.”
According to the trooper, Crawford refused to complete a seated field sobriety test and refused to consent to a blood draw to test his blood alcohol level.
The affidavit states law enforcement continued their investigaiton and were able to obtain a search warrant for Crawford’s medical records from both Muskogee EMS and the hospital where he was treated.
The affidavit outlines that records from Muskogee EMS stated, “CRAWFORD did not remember how he got out of the vehicle…CRAWFORD stated he remembered the impact; however, he did not recall leaving the vehicle. The narrative noted CRAWFORD to have a presumed loss of consciousness.”
Court documents go on to state that Muskogee EMS noted Crawford seemed exceptionally confused at the scene.
The affidavit states, “Crawford thought it was 2016 multiple times and when told it was not 2016 but 2026, he kept repeating that 2016 and 2026 were the same…Crawford kept insisting he was near Catoosa, when EMS told him he was not near Catoosa but south of Haskell, CRAWFORD stated they were lying to him.”
Court records show that the medical records obtained from the hospital stated Crawford had an alcohol level of 0.25 at the time his blood was drawn.
Law enforcement had a warrant issued for Crawford’s arrest on Wednesday. Authorities said Crawford later willingly surrendered himself to the Tulsa County Jail.
Following the tragic crash, the community has been in mourning for the loss of Isabella Clay.
Clay was a member of the Coweta FFA and served on the 2025-2026 Agriculture Youth Council.

On Monday, Oklahoma Governor Kevin Stitt released the following statement on social media about Clay’s passing:
“My heart goes out to the family and friends of Isabella Clay, a high school student who tragically lost her life in a crash this weekend.
Isabella was a dedicated young leader in Oklahoma agriculture and served on the 2025-2026 Agriculture Youth Council.
I am praying for peace and comfort for her loved ones and community as they navigate this heartbreaking loss.”
The Coweta FFA also released the following statement:

“These words are hard to type but we unfortunately lost a member of our chapter this morning. Isabella Clay was involved in a car accident last night and has joined God’s flock. She loved the FFA and touched the lives of everyone she met. She was a true example of what FFA means and will be dearly missed. Please keep her family in your prayers and thoughts.”
