New Saint Francis program brings hospital-grade treatment into patients’ homes

TULSA, Okla. — A new program designed to bring hospital-level care to patients at home is on its way, courtesy of Saint Francis Health System. 

“This program expands where hospital-level care can happen for the right patients,” said Dr. Cliff Robertson, President and CEO of Saint Francis Health System. “For some patients, the safest place to heal can be at home — while remaining fully connected to Saint Francis care teams.”

Saint Francis at Home is for those who have medical needs that are able to be safely treated outside of the hospital. While it does not replace in-hospital care. It allows patients more options. 

“This initiative represents a significant step forward in our mission to extend the presence and healing ministry of Christ,” said Dr. Reetu Singh, Chief Medical Officer for Saint Francis Health System. “This aligns seamlessly with our vision for a healthier tomorrow and to meet our patients where they belong.” 

Physicians identify a patient’s eligibility based on clinical criteria and safety. Patients are closely monitored and providers deliver medical care in the home with hospital-grade equipment and standards. 

Care is coordinated and overseen by Saint Francis personnel and a nurse navigator follows up with discharged patients for two weeks. 

“High-quakliy care shouldn’t be limited to a hospital room when the same level of care can be safely delivered at home, for the right patients, at the right times,” said Dr. Robertson. 

Saint Francis partnered with DispatchHealth to deliver in-home care with support from CommunityCare to provide insurance courage options for patients. 

“At DispatchHealth, we are a home care agency. We come into the home, take care of people there while they’re in their own environment. That way they can stay home and get better,” said Brandi Dickinson, Clinical Nurse Lead of the Tulsa Office for DispatchHealth. 

Patients can receive the following with Saint Francis at Home: 

  • Daily APP/physician visit 
  • RN visits and 24/7 virtual RN access 
  • Primary and specialist provider coordination 
  • Respiratory therapy 
  • Physical therapy 
  • Occupational therapy 
  • Lab work 
  • Pharmacy services 
  • Imaging 
  • In-home ER as needed 
  • Transportation 
  • Meal delivery 

Following care, patients who are in a transition period and have a dedicated nurse navigator can receive the following services: 

  • Plan of care 
  • Medications 
  • Follow-up appointments 
  • Readmission prevention 
  • Patient education 
  • Home health, as needed 
  • In-home ER, as needed
